Primary Health Care in Southwest Louisiana
Since 1993, Southwest Louisiana Primary Health Center has been providing affordable outpatient medical and dental healthcare services to our community which includes St. Landry Parish, Northern Lafayette Parish, and Evangeline Parish.
In addition, we will assist the citizens of St. Landry and surrounding parishes in developing their greatest potential and assisting them to attain a purposeful life through education, family support, and community resources.
We are a non-profit corporation governed by a Board of Directors.

Mission: To deliver comprehensive quality health care services by improving the accessibility, availability, and acceptability of such services to the people within the service area.
Vision: In partnership with the community, we will create, develop, and promote supportive service to all people who are medically underserved or needy, so that they may experience all the rights, privileges, and responsibilities as members of the community.

Evangeline Family Medicine Center
Since July 2009, Evangeline Family Medicine is now open in Ville Platte, Louisiana providing medical and counseling services.
